IMPORTANT ACTION DUE: MARCH 31, 2022

Please remember to assign up to a maximum of 15 attendees (including all students, teachers, mentors, and anyone else affiliated with the team) from your team who will be attending the event.  Here is what we need from you

  1. Please complete the Spreadsheet Template here
  2. Complete the Team Roster on the FIRST Inspires Dashboard and download the team roster

Registration and Load In Map

SCREENING AND REGISTRATION REQUIREMENT FOR ALL ATTENDEES:

Required documentation (all participants):

  • Ontario Enhanced Covid Vaccine Certificate (QR Code)
  • ID (photo ID not required)
  • Completed Provincial COVID-19 Self-Assessment

Symptom Screening (all participants):

All  attendees are required to complete the following Provincial screening questionnaire prior to arriving at the event: https://covid-19.ontario.ca/screening/worker/

Check-in:

Please check-in at one of the two following locations (see map below):

  • Load-in team:  St. Mary Cafeteria Doors (back of school)
  • Other team members: St. Mary event “Main” entrance (back of school)
  • Volunteers: St. Mary event “Main” entrance (back of school)
